All student athletes who participate in Peabody-Burns Junior-Senior High School sports must have a current physical before they are allowed to practice. The physical must be completed after May 1, to be valid for the 2009-10 school year.
The physical form provided by the school district must be used, and pages 1, 3, and 4 on the form must be completed and signed by the student athlete and parent or guardian before a physical can be obtained. Physical forms are available at the PBHS office.
Physicals will be available 4 to 6 p.m. Tuesday at Axtell Clinic, 700 Medical Center Dr., Suite 210, Newton. No appointment is needed. The cost is $20 with $10 per physical returned to the school.
Fees are payable at the door, and no insurance paperwork will be filled out.
Parents or guardians also have the option of getting student physicals from family physicians who can complete any necessary paperwork regarding insurance. If the parent elects to use Axtell Clinic and wants to file for insurance benefits, the student or parent must make an appointment rather than taking advantage of the walk-in date and time.
